A common hurdle with EyeEase (and many Envato kits) is the appointment form. After you import the page, the form won't appear immediately. You must:
Import the MetForm block separately in the kit settings.
Edit your Appointment Page with Elementor.
Click the MetForm widget and select "Edit Form."
Choose the EyeEase MetForm template from your library to "Insert" it.
